PATIENT INFORMATION
Appointments
At Chanhassen Dental we value our patients time, and that you respect ours. When you make
an appointment at our office, that time is exclusively reserved for you. If, for some reason you
are unable to make this reserved time please contact us at 952-934-3383 with at least a 24 hour
notice. We will be happy to help you reschedule to a better time that works best for your
schedule. We reserve the right to charge a cancel/no show fee of $50.00 if within the 24 hour
notice window.
We do send both email and text confirmations, once you have confirmed your appointment you
will not receive another confirmation reminder until the day of your appointment.
Dental Health Insurance
We are happy to file your insurance claims on your behalf, and you will find that we are in
network with several dental insurance plans. It is always best to check with your own insurance
plan to verify your benefits. Please make sure to update your insurance information with us, so
that we may submit to the correct insurance company to help us to utilize your benefits. We
understand that dental benefits can be confusing, and are happy to help you navigate and
maximize your benefits, so you can get the treatment you need.
Payment Information
We accept cash, personal checks, American Express, Discover, MasterCard, Visa, and Care
Credit cards for payment. We appreciate your full payment or if you have dental insurance
down payment towards services rendered.
Finance/payment plan options:
We understand some necessary treatments can be expensive, and are happy to offer Cherry if
you are in need of doing a payment plan for your treatment. Please see Payment Plan tab for

Statements
Chanhassen Dental sends monthly statements to our patients. We ask that you pay in full by
the due date on your statement. If you have dental insurance you will see and astrik (*) by the
procedures that have been sent to insurance. If you do not see payment from insurance,
please know we are just waiting on payment from your insurance company. Any questions that
you may have on your statement please reach out to our office at 952-934-3383. We do have
the ability to send statements by email or text as well as mail. If you would like to receive your
statement by email or text please let us know.
Patient Forms
We understand that our patients have busy schedules! That is why we offer online patient forms. If you are a new patient to our office please click on New Patient Portal to fill out your information online, or New Patient Registration Form to print a paper form to fill out. Please bring your insurance card, and forms with you to your first dental appointment at Chanhassen Dental. If you would like us to request records from another office, please click on the Dental Record Release Form. Feel free to email this form to our office (office@chanhassendental.com). We will be happy to contact your previous office for you.
