PATIENT INFORMATION
Appointments
At Chanhassen Dental we value our patients time. When you make an appointment at our office, that time is exclusively reserved for you. If, for some reason you cannot make your appointment, please contact us as soon as possible so that we can reschedule your appointment and offer your time slot to another patient.
Pre Med
If you have artificial joints (Hip, Knee replacement, etc.) or a heart condition please let us know if you need to take an antibiotic for your dental work, we can help remind you when confirming your dental appointments.
Payment Information
We accept cash, personal checks, American Express, Discover, MasterCard, and Visa. We appreciate your full payment, or down payment at the time of services rendered. If you would like paperless statements, we have options for your to receive by email or text. You have options to pay online as well as using Visa, Mastercard, Discover, and American Express. Dental Health Insurance
We are happy to file your insurance claims on your behalf, and you will find that we are in network with several dental insurance plans. We understand that dental benefits can be confusing, so our front desk team members are here to help you navigate and maximize your benefits so you can get the treatment you need. Chanhassen Dental will send statements to our patients once your insurance has paid their portion. We request payment in full by the due date on your statement. Patient Forms
We understand that our patients have busy schedules! That is why we offer online patient forms. If you are a new patient to our office please click on New Patient Portal to fill out your information online, or New Patient Registration Form to print a paper form to fill out. Please bring your insurance card, and forms with you to your first dental appointment at Chanhassen Dental. If you would like us to request records from another office, please click on the Dental Record Release Form. Feel free to email this form to our office (office@chanhassendental.com). We will be happy to contact your previous office for you.
